【摘要】 目的: 觀察缺血前和再灌注前葛根素(PUE)治療對(duì)大鼠心肌缺血-再灌注損傷(I/R)的保護(hù)作用。方法: 開(kāi)胸結(jié)扎左冠狀動(dòng)脈前降支(LAD)復(fù)制大鼠心肌I/R模型,設(shè)立假手術(shù)(sham)組、I/R組、PUEA組(缺血前5 min給藥)和PUEB組(再灌注前5 min給藥),比色法測(cè)定血清肌酸激酶(CK)活力,放射免疫法測(cè)定血漿內(nèi)皮素(ET)濃度,硝酸還原酶法測(cè)定血清一氧化氮(NO)濃度,TTC法測(cè)定心肌梗死面積。結(jié)果: 與I/R組比較,PUEA、B兩組血清CK活力明顯降低,血漿ET濃度明顯降低,血清NO濃度明顯增高,心肌梗死面積明顯縮小(P<0.05),且PUEA、B兩組間比較差異無(wú)顯著性(P>0.05)。結(jié)論: 缺血前和再灌注前葛根素治療均能明顯減輕大鼠心肌I/R損傷,且兩種給藥時(shí)機(jī)的保護(hù)效應(yīng)相近。

Protective Effect of Puerarin Preconditioning on Myocardial

[Abstract] Objective: To investigate the protective effect of puerarin (PUE) before ischemia or before reperfusion on myocard against ischemiareperfusion injury in rats. Methods: Rats myocardial ischemiareperfusion injury (I/R) were induced by ligating the left anterior descending coronary artery for 45 minutes and reperfusion for 120 minutes. Twentyfour rats were randomly divided into four groups (n=6 each): shamoperation group (group S), ischemiareperfusion group (group I/R), PUE treated groups (group PA received PUE at 100 mg/kg intravenously 5 minutes before ischemia, and group PB received PUE at 100 mg/kg intravenously 5 minutes before reperfusion). Serum creatine kinase (CK) activity was measured with colorimetric method, concentrations of plasma endothelin (ET) detected with radioimmunoassay, and plasma NO content measured with Griess reaction assay. TTC dyeing was used to determine myocardial infarction sizes. Results: CK activity and ET concentrations were significantly lower, and the serum NO contents were significantly higher in groups PA and PB than in group I/R. Myocardial infarct sizes decreased dramatically in groups PA and PB, and there was no significant difference in all of the indexes between the 2 groups. Conclusion: Preconditioning with puerarin before ischemia or before reperfusion can reduce myocardial ischemiareperfusion injury in rats, and the protective effects of puerarin injection at these 2 time points are similar.

缺血預(yù)處理(ischemic preconditioning,IPC)可對(duì)缺血-再灌注的臟器產(chǎn)生明顯的保護(hù)作用,但臨床上難以實(shí)施,因此藥物預(yù)處理(pharmacological preconditioning, PPC)正逐漸被人們?nèi)找骊P(guān)注。葛根素對(duì)心肌缺血-再灌注損傷(ischemiareperfusion injury,I/R)的保護(hù)作用已被大量資料證實(shí)[1~4]。本課題組也曾報(bào)道葛根素預(yù)處理對(duì)心肌I/R能產(chǎn)生與IPC相似的保護(hù)效應(yīng)[5],葛根素的保護(hù)效應(yīng)可能與線粒體ATP敏感性鉀通道開(kāi)放有關(guān)[6]。2004年3月~2005年1月,采用左冠狀動(dòng)脈前降支(left anterior descending coronary artery,LAD)結(jié)扎復(fù)制大鼠心肌I/R模型,分別在缺血前和再灌注前給予葛根素治療,比較不同時(shí)機(jī)給予葛根素對(duì)I/R心肌的保護(hù)作用,為臨床應(yīng)用葛根素提供實(shí)驗(yàn)依據(jù)。
